Typically what is needed for an admission would be complete medical records from your primary care physician. These records will provide a better overview of your current health status and your healthcare needs. Also, based on your records, our care team can get the required items to meet your needs such as oxygen tank, nasal cannula, c-pap, air loss mattress etc… We can accomplish an easier transfer this way.
See Your Primary Care Physician
We recommend a visit to your primary care doctor in order to know your current health status. It is very important to visit your primary care doctor especially to get a routine checkup, basic blood work, or to have your medications regulated.
Insurance Documentation Required
Please provide proof of Medicare, Medicaid and any other long term care insurance. Please call our Admissions Coordinator for detailed information about the admissions process.
